- Family doctor
- Some outpatient clinics
- Some local health departments
- Some schools
WHEN DO TEENS GET CHECKUPS?
If you are under 21 and your family is eligible for Medicaid, you can get a
complete checkup at ages:
- 14 years
- 16 years
- 18 years
- 20 years
Or you can get a checkup every year if your doctor says you should. When your
doctor checks you out,
you will get lab tests if you need them and get started on early treatment if you
have any medical
problems
This program will also provide dental care, nutrition information and alcohol or
drug counseling.
Medical reports can be used for:
- job applications
- sports
- any school activity that requires a physical exam (with a signed Release of
Information form)
Health Services Schedule
Appointments for child health screening examinations should be scheduled for
children, teens and young
adults at the following ages:
- Infants - Newborn, 2 months, 4 months, 6 months, 9 months, 12
months.
- Age 1 to 2 -- two times.
- Age 2 through 5 -- one time each year.
- Age 6 through 20 -- one time every other year.
- Additional child health screenings can be provided when
necessary.
Shots should be given at:
- 2 months
- 4 months
- 6 months
- 15 months
- 18 months
- 2 years
- 5 years
- 14 years
